Food law is a third official bookshelf. Hemp seed, seed oil and seed flour are old foods in the EU, not novel; Regulation (EU) 2023/915 sets THC maxima (3.0 mg/kg seed; 7.5 mg/kg seed oil). CBD isolates and hemp extracts are novel foods; EFSA’s 2022 statement listed data gaps and has not cleared a general food use. Human evidence for Cannabis sativa is strongest where a named, assayed product met a regulator—not where a gas-station bottle borrowed the Latin name.
Cannabidiol oral solution (Epidiolex / Epidyolex) is plant-derived, highly purified CBD. GWPCARE3 and GWPCARE4 (Lennox–Gastaut) and GWPCARE1 and GWPCARE2 (Dravet) were randomized, double-blind, placebo-controlled add-on trials: median drop or convulsive seizures fell versus placebo, with dose-dependent liver-enzyme and somnolence warnings on the label. Tuberous sclerosis complex was added later. That is a medicine, with a therapeutic dose far above novel-food guesswork.
Nabiximols (Sativex) is an oromucosal spray of a cloned-sativa extract standardized to roughly 1:1 THC:CBD plus minor cannabinoids. Adult MS-spasticity RCTs and a 2023 meta-analysis of seven trials (1,128 people) support add-on use when standard antispasticity drugs fail, mostly on patient-reported NRS. A 2024 objective-tone study (RELEASE MSS1, MAS LLMT-6) missed its primary endpoint—an honest reminder that spasticity is hard to measure. A pediatric cerebral-palsy nabiximols RCT did not beat placebo on NRS and recorded hallucinations. Approval exists in many countries; it is not a U.S. FDA MS label in the same way Epidiolex is an epilepsy label.
Dronabinol (synthetic Δ9-THC) and nabilone (a synthetic analogue) have older FDA indications for chemotherapy nausea and, for dronabinol, AIDS anorexia. A 2022 pharmacology-sorted meta-analysis of 152 cannabinoid RCTs (12,123 participants) found high-grade evidence for CBD in those epilepsies, moderate evidence for nabiximols on adult MS pain/spasticity/sleep, and more modest, indication-specific signals for dronabinol. Whole-flower “medical cannabis” observational cohorts are larger than their controlled trials; they are not Ph. Eur. 3028 plus a Phase III number.
Hemp seed as Huo Ma Ren has traditional and food-use human experience as a moistening laxative; it does not have Epidiolex-grade seizure RCTs, and the seed is not the flower.
Important cautions, held with the plant: THC is intoxicating and a controlled substance in most jurisdictions; high-dose CBD can raise transaminases and interact with clobazam and other CYP substrates; pregnancy, driving and adolescent brain data cut against casual use; CBD food is not authorized in the EU while assessment gaps remain; a 0.3% agricultural cap is not a medical dose; this overview is not cultivation, extraction, or dosing advice.
